How do I request a copy of an investigation or incident report? To request a copy of an investigation or incident report, you will need to provide ONE of the following:

  • Your name, date and time of incident, address or location of incident, type of incident (vehicle fire, building fire, vehicle accident, etc.), OR
  • The Fire Department incident number
